Patient ID Cards

Requirements:

First Requirement: Please READ the following:

  • You must be a current patient in the Puerto Rico Department of Health’s Medical Cannabis Program. The license is printed directly from the Health Department portal by validating your date of birth and the last 4 digits of your Social Security number. If you do not wish to provide this information, we understand, but we will not be able to provide the service without it.
  • All communication takes place in writing (via WhatsApp or email)—we do not accept voice calls. This allows us to maintain a clear record of every order and provide accurate information.
  • Important: Only the patient—the person named on the document—may place the order.
  • Make the payment corresponding to the type of card you wish to order.
  • You will receive your card by mail in approximately 3–5 business days.
